2007 Nissan Quest S problem?

Hey guys, I have a 2007 Nissan Quest S I have been having some trouble out of. I was wondering if you guys could help me figure out what is wrong.

Here are some Symptoms:

1. When I press the gas lightly, there is a grinding sound. If I push the gas harder it seems to go away.
2. Around 65 mph there is a vibration from the front end (not unbearable)
3. Car doesn't seem to handle bumps in the road as well as it should consistently.
4. Tires keep going out of balance.

I have replaced the brakes and rotors in the last six months, but it seems my braking is once again is rough. Car shakes as I brake. I have thought about motor mounts but it doesn't seem to have all of the symptoms.

Are these the same problem or separate problems? Does anyone have an idea? Thank you in advance for your help.
